Youth in Transition Program

The Youth-In- Transition Worker Program offers 1:1 support and guidance to youth leaving the Children’s Aid Society care as they identify, access, and navigate adult service systems. Youth-In- Transition Workers will be available to meet regularly with participants to provide life skills coaching, information and referrals, and offer guidance and mentoring as youth access supports, and can assist youth in speaking on their own behalf.

Eligibility

  • Youth 16 to 24 years of age
  • Transitioning out of the care of a Children’s Aid Society
  • Eligible for Renewed Youth Supports at age 16 or 17 (whether or not supports were accessed)

When necessary, the Youth-In-Transition Worker will assist in determining eligibility.
